Please print and complete the following forms except for the records release to help expedite your first visit: The records release is only needed if you are requesting us to either send or recieve your medical records.
**Effective Jan 1, 2013 all patients will be required to bring in their insurance card and update all necessary paperwork on their first visit of the year.
